Adult Services

Medicare Home Health Programs -

  • Wound Care Program
  • Diabetic Education Program
  • Geriatric Rehabilitation Program
  • Congestive Heart Failure Program

Wound Care Program -

  • Weekly Wound Assessment & Reporting.
  • level II Trained Wound Vac Nurses.
  • Light Therapy available as an adjunct treatment to physician's wound care protocols. Treatments administered by trained nurses or therapists.

Diabetic Education Program -

  • Non-Insulin Dependent Diabetes - Three-week intensive training including diet plans, exercise, pancreatic function, foot care, glucometer testing.
  • Insulin Dependent Diabetes - Six-week intensive training including the above three-week instruction plus teaching insulin types, preparation, and injection.
  • Coordination of glucometer and testing supplies.
 

Geriatric Rehabilitation Program -

  • Joint Replacement - Knee, Hip, and Shoulder Rehabilitation
  • Balance Improvement - Reduce falls/injury
  • Neurological Rehabilitation - Improving functional independence & level (stroke, multiple sclerosis)

Congestive Heart Failure Program -

  • Teaching cardiac function, symptom management, essentials of medication, diet pitfalls, and life enhancement through exercise.
  • Cardiac rehab through physical therapy, if appropriate.

CareWell -

CareWell is a cost-effective program that utilizes highly trained unlicensed caregivers to provide care for patients who wish to remain in an independent living environment. We start with a thorough evaluation by an RN who will determine the client's health status and medical needs. The RN will determine if the client's needs can be met by an unlicensed caregiver based on the stability of the client's status, the nature of the tasks, and the training of the caregiver. The RN will develop a plan of care for the patient including the frequency of supervision required.

Care for complex patients including those with:

  • Spinal Cord Injuries
  • Traumatic Brain Injury
  • Congenital Disorders
  • Motor Vehicle Accidents

RN Delegation of Duties to Highly Trained Attendants Can Include:

  • Intermittent Catheterization
  • Bowel Program
  • G-Tube Care and Feeding
  • Meds Administration
  • Trach Care
  • Suctioning
  • Ventilator Support

In-House Medical Equipment -

  • Respiratory Equipment and Service Program
  • Home Durable Medical Equipment and Delivery.
