CareWell, the personal Assistance Services division of The Care Group, was developed in response to patients who need assistance with health care and personal needs and wish to remain in an independant living environment.

The CareWell Program provides a cost-effective alternative to skilled care by utilizing highly trained caregivers as an integral part of our healthcare delivery system.

We start with a thorough evaluation by a Registered Nurse (RN) Case Manager who will determine the patient?s health status and medical/personal care needs. The RN will determine if the patient's needs can be met by an unlicensed caregiver based on the stability of the patient's status, the nature of the tasks, and the training of the caregiver. The RN will develop a plan of care for the patient including the frequency of supervision required.

• Dementia
• Post-Surgical Recovery

Duties of Highly Trained Attendants Can Include:

• Intermittent Catheterization
• Bowel Program
• G-Tube Care and Feeding
• Trach Care
• Suctioning
• Ventilator Support

Additional Personal Care Services Include:

• Personal Hygiene
• Medication Management
• Light Housekeeping
• Meal Planning/Preparation
• Safety with Ambulation/Toileting
• Errands ? Medical Visits/Shopping
