The Care Group would like to assist with your discharge planning
needs for patients requiring home respiratory services. This includes
adult and pediatric:
- Oxygen
- CPAP/Bi-PAP
- Home ventilators
- New or existing tracheostomies
- Suction machines
- Apnea Monitors
- Infant sleep studies
- Pulse Oximetry
- Nebulizers
Our goal is to provide patient and caregiver education prior to
the discharge. For new ventilator and new tracheostomy patients,
it is important that we have as much advance notification for teaching
as possible.

Guidelines for the following complex discharges:
- Home Ventilator patients
To help ensure a safe discharge, please notify The Care Group 5-7 business days before anticipated
discharge. We then schedule an initial patient evaluation at your
facility. After obtaining insurance authorization, education classes
and a home safety evaluation are scheduled with the patient and
caregiver. The caregiver must demonstrate knowledge of equipment
prior to the patient’s discharge. It is preferable that
the discharge occur in the early part of the week such as Monday
through Thursday.
- New Tracheostomy patients
Please notify The Care Group with as much advance notice as possible before anticipated
discharge. After obtaining insurance authorization, education
classes and a home safety evaluation are scheduled with the patient
and caregiver. The caregiver must demonstrate knowledge of trach
care and equipment prior to the patient’s discharge. It
is preferable that the discharge occur in the early part of the
week such as Monday through Thursday.
Guidelines for other Respiratory Services:
- Home Oxygen
For Medicare patients, a qualifying O2 Sat at 88 or below on room
air or a PAO2 at 55 or below is required. Our Respiratory Department
will deliver a portable unit to your facility and will coordinate
home delivery of stationary oxygen and supplies. Our Respiratory
Therapist will do an evaluation after home oxygen is set up.
- CPAP/Bi-PAP
An appointment with our Respiratory Therapist is scheduled for
the patient to come to our office for set up, which includes education, equipment instruction, and mask fitting. Other arrangements can be made if the patient has
special needs.

- Apnea Monitors
Our Respiratory Therapist will provide apnea monitor education
classes and reinforce CPR training at your facility prior to discharge.
